Lending Library Request Form

We are a local charity that provides quality care & support to birthing women & their families. Instead of charging a rental fee, we kindly ask that you would consider making a donation based on what you can afford.

This donation is not necessary, but we do rely on the generosity of community members to continue our work, and would be very grateful.  ALL funds remain local and help us to impact our community in beautiful way. 💜 Yes, I'd love to donate!

QueenCare’s Lending Library is committed to promoting the safety and well-being of mothers and their infants. Users are advised to carefully inspect ALL borrowed baby equipment, adhere to manufacturer guidelines, and ensure proper installation. QueenCare assumes no liability for any accidents or injuries of any nature whatsoever, including but not limited to physical and financial injury, resulting from the use of borrowed items.  In using the lending library, the borrower agrees to forever release and discharge QueenCare and its representatives from any and claims which the borrower, its heirs, successors, and assigns may have against QueenCare which are related to or arise out of the borrower’s use of the lending library, including but not limited to those allegedly attributed to the negligent acts or omissions of QueenCare. It is the responsibility of the borrower to seek professional advice if unsure about the correct usage or safety of any equipment.
