Photo and Video Release Form

Dear Student,

From time to time, NORMI staff may take photographs or video recordings during class for educational and promotional purposes. We would like your permission to use these images and/or recordings, which may include your likeness, in materials such as newsletters, websites, training resources, and social media posts.

Permission Statement

I, the undersigned, hereby grant permission to NORMI to record, use, and publish photographs and/or video footage of me taken during class sessions. I understand these materials may be used for educational, promotional, or informational purposes in print and/or digital formats.

I understand that:

• My image may be used without further notice or compensation.

• My name may be used in association with these materials.

• I may revoke this permission in writing at any time.
