This application is for individuals seeking financial sponsorship to live in a sober living home with support from the Phoenix Recovery Foundation.
PRF sponsorship is reserved for people who are ready—ready to take ownership of their recovery, actively work a plan, and contribute to a stable, growth-oriented environment. Please answer honestly and thoroughly. Every question helps us understand how we can best support your next chapter.

Tell us about the financial challenges you’re facing and why outside support is necessary right now.
We understand that every recovery journey is different. Some people may need a few months of support, others a year or more. There’s no pressure to “have it all figured out,” but we ask for your honest estimate. This helps us plan ahead and give each person the best possible chance at long-term success.
This could include work, education, recovery efforts, volunteering, parenting, or anything else you're putting your energy into.
We don’t expect you to have it all figured out, but share how you’re working toward income, stability, and independence. What are the next steps?
By requesting sponsorship through the Phoenix Recovery Foundation, you are committing to the following expectations that are grounded in science and proven to support long-term recovery:

1. Participate in Mutual Support Groups
You will attend at least 2 support meetings each week (such as AA, NA, SMART Recovery, etc.) and provide simple proof of attendance (e.g. signature sheet, selfie with speaker, etc.).

2. Stay Engaged with a Professional Support Person
You will stay actively connected to a case manager, probation officer, peer recovery specialist, or therapist who is involved in your progress and reachable if needed.

3. Maintain a Daily Structure
You will keep a consistent weekday schedule by working, volunteering, or actively job-seeking—and you’ll submit and update your schedule weekly.

4. Create and Follow a Written Plan for Self-Sufficiency
You will write out a personal plan for how you’ll eventually support yourself and transition out of sponsorship. The timeline is flexible—but the effort is expected.

5. Meet the Minimum Sobriety Requirement
You must have at least 30–60 days sober prior to entering the house, unless you’re coming directly from inpatient or detox and have a follow-up care plan.

6. Engage in a Weekly Pro-Social or Community Activity
You’ll participate in one weekly activity that connects you to something bigger—this could be a house dinner, volunteering, church, or anything that builds healthy connection.

7. Contribute to the House Community
You will attend weekly house meetings, participate in chores, and do your part to help the house stay peaceful, safe, and well-maintained.

8. Respect House Rules and Conflict Procedures
You agree to follow the written house rules, drug testing policy, and the agreed process for conflict or relapse—including the consequences outlined in the program.

9. Consent to Random Drug & Alcohol Testing
You agree to submit to random urinalysis or breathalyzer tests as part of maintaining a safe, sober environment for all residents.

10. (Optional but Encouraged) Limit Digital Distractions
You are encouraged to reduce excessive phone or social media use—especially after 10PM or in common areas—to help improve sleep, reduce anxiety, and stay focused.
