For Churches or Ministry Organisation applying for Affiliation with Crosslink. Once affiliated, organisations can nominate their pastors/leaders for Endorsement as a Minister. If you cannot complete this form online, you can print the form, or contact us for a PDF version.

Please DO NOT send payment with your application form. If the application is approved, a Tax Invoice for annual fees (pro-rata) will then be issued to you.
